For Coaches:

Some important items:

• If you have Manager or Assistant Coach/Coach In Training - then they will need to submit the appropriate form for the Executive meeting on Monday nights. These forms are located at about us: useful forms. 
• Once form completed: Email to bmna@bmna.com.au and copy in Denise Thrift.
• Each player needs to receive these forms:    a) Medical Information Form,  b) Information Form.
• The Medical Form should be completed and returned to your Team Manager asap.  The Team Manager then needs to retain all of these.
